Families of Student Athletes – Medical Forms and Information

All Student-Athletes are required to submit medical forms prior to receiving a sports physical or starting practice in the fall. All students will have an account created on Healthy Roster, which is where the forms can be completed and submitted online. Students will also be issued codes to complete ImPact Concussion testing (also online) to meet the requirements of the NCAA concussion policy. Forms and codes will be issued in early July.

Sickle Cell Trait Information

All incoming (first-year and transfer) student-athletes are required by the NCAA to submit Sickle Cell Trait status prior to participation. You can either obtain your test results at birth or get a new blood test (tell your physician you need a Sickle Cell Trait Screen) and submit the results to your Healthy Roster account.
