(If you have one of these plans, we will bill your insurance, and your vaccine will be free.)<\/p>\n<p><strong>Payment <\/strong>(if not using insurance): Cash or Card.<\/p>\n<ul>\n<li>Cost for ages 6 mos. to 64 years is $25.<\/li>\n<li>Cost for those 65 years and older (high dose) is $45.<\/li>\n<\/ul>\n<p>Please review this<strong> <a href=\"https:\/\/sites.owu.edu\/dailymonster\/wp-content\/uploads\/sites\/191\/2017\/09\/Vaccine-Consent-Form.pdf\">vaccine consent and information sheet<\/a>\u00a0<\/strong>and<strong> bring a signed copy to your vaccination appointment. <\/strong>(We will have copies on site, if needed.)<\/p>\n<p><strong>If you are unable to attend our Sept. 29 clinic<\/strong>, you are invited to contact the Health Center to make an appointment for another time.<\/p>\n<p>Please note: Flu vaccines available from Delaware General Health District this year are the quadrivalent vaccines and the high-dose vaccines.
