Student Requirements:
•Complete an application and submit by the May 15.
•Furnish an official transcript and a faculty member letter of recommendation to the Columbus Chapter’s Scholarship Committee.
•Deliver all requested documents to the Columbus Chapter’s Scholarship Committee, P.O. Box 1265, Hilliard, Ohio 43026 on or before the deadline.
